How do I use the tileset editor?

I know this (might) be a stupid question, but...
The built-in tileset editor is so confusing, I don't even know how to put in a block. So could anyone help me by chance?

If this were added, it would kind of take away the point of Mario and Luigi only having item boxes, as mostly everyone would use them so their custom HUDs would support them. But if you mean keep the heats, then that might be of use.
